Why we do this: When a machine a tech built comes back with a problem, it counts as a −1 against the tech who built it. Most failures post on their own from swap and test-fail tickets. Use this page only for a failure the system missed. Every entry is checked for duplicates, waits for Steve or Nikki to approve, and is never deleted.
